All attempts to make an outgoing call result in a fast busy. Our VOIP service is via Mitel and they have stated they have 'refreshed' the line and provided me with a user name and passcode to enter into the Polycom unit but I have no idea what they are talking about... Is this a Mitel or Polycom issue?

The SoundStation 2 is an analog phone which does not require a username / password.
This sounds more like a SIP phone or ATA adaptor.
The SoundStation 2 requires a normal analog line and what you describe is an issue on the phone line provider end.
