Hi, theres no answer for my question and this post cannot be accept as solution,

The unit is running ok but i'm afraid it will comes again as there's no confirmation

what the cause of issue.

the post was replied to by other users which the solution I provided, aka checking the release notes prior to upgrading and following the outlined path, fixed their issue.

 

I have no internal Poly reference for your case so I am unable to comment if this ever came into Poly.

If this happened due to a power fail, I would format and zeroize it to see if that helps. If not, it is probably a hardware issue card is probably bad. 

 

For those who have has this happen during the upgrade to 6.2.x, I find that if I manually power it off with the static button and quickly back on, it picks up where it left off

 

When I upgraded to 6.1.x, a few systems did get bricked.
