Troubleshooting an audio issue can be very tricky. My first guess would be something network related like packet loss or excessive jitter. This can usually be remedied by implimenting QOS and Traffic shaping.
In order to know for sure, you would need to perform a packet capture using a program like Wireshark. Once you have captured the traffic, you can perform an RTP analysis to check for things like delay, jitter and Packet loss.
