My Environment:
Office 365 Enterprise E3 Plan
Lync 2013 (15.0.4605.1000) MSO (15.0.4605.1002) 64-bit, Part of Microsoft Office 365 ProPlus.
Microsoft Lync Enterprise Voice services (hosted by 3rd party)
CX3000 IP conference phone (Optimized for Lync 2013)
CX3000 Software: 4.0.7577.4420
I have successfully signed into the phone using Enhanced mode (i.e. via USB), I cannot sign in using Basic Mode (i.e. ext/or phone # & PIN).
Q: Does Basic Mode sign-in work with Office 365/Lync? If so how can I get detail configuration setting for this feature?
Did not find any Admin/set-up instructions here (just general end user instructions): http://support.polycom.com/PolycomService/support/us/support/voice/cx/cx3000.html

To my knowledge Pin & Extension requires Option 43 at least for a normal en premise installation.
