Hello Dave,

The initially reported issue that I am aware of was fixed in UCS 4.0.4

 

It should be in the release notes as:

 

  • VOIP-76769 Incoming call missing SIP DISPLAY INFO does not show in missed / received calls

Anything else within this post that does not cover the above needs to be logged via the Polycom reseller so Polycom support can look at it.

Found it and also tested myself - can confirm that this bug is now resolved in 4.0.7
