The initially reported issue that I am aware of was fixed in UCS 4.0.4
It should be in the release notes as:
- VOIP-76769 Incoming call missing SIP DISPLAY INFO does not show in missed / received calls
Anything else within this post that does not cover the above needs to be logged via the Polycom reseller so Polycom support can look at it.
