[Zero Touch Provisioning] My csv does not appear to be uploading

I have tried uploading a csv with a test phone in it but I cannot find it when I search or it using the reference ID. Are there any known issues using the bulk add feature on ztp? Can someone provide an example of a csv file for bulk upload to make sure mine looks correct? I followed the manual for ZTP but no luck so far. Any ideas would be appreciated.
